Expense Claim Form

The Daily Living Allowance claim form enables you to claim £55 per day you are away in the UK or £75 per day you are away in the US or Europe for treatment or therapy.

To Claim the Daily Living Allowance we will need verification of dates from your service provider direct to us and made out to Tree of Hope with your Child’s full name as a reference. Please submit all paperwork at least two weeks before you travel to ensure funds are paid in time for the Hospital/reason for trip.

• Please use separate line for each travel date in the box below.
• Please state starting point and destination for each Trip.
• Only submit forms with accurate mileage calculations. The accounts team will spot check forms to ensure the mileage stated on claims is correct.
• If you are not taking a direct route, please ensure you make this clear on your form, as our spot checks will be done using Google Maps and will not take into account any unstated detours.
• Mileage to be claimed at 45p per mile.
